Abstract

"In many ways, primary care in Denmark performs well. Danish primary care is trusted and valued by patients, and is relatively inexpensive. But there are important areas where it needs to be strengthened. Most critically, Danish primary care is relatively opaque in terms of the performance data available at local level. Greater transparency is vital in the next phase of reform and sector strengthening. Robust information on quality and outcomes empowers patients and gives them choice. It can support GPs to benchmark themselves, and engage in continuous quality improvement. It also allows the authorities to better understand where they should direct additional resources. This report draws on evidence and best practice from across OECD health systems to support Denmark in: agreeing on the steps that will strengthen its primary care sector, delivering high-quality, patient-centred care, and establishing a sustainable footing as the foundation for a high-performing health system."--Back cover.

Abstract

This report puts forward policy recommendations for strengthening the performance and sustainability of the health care system in Costa Rica. There is much to praise in Costa Rica's health care system: institutional stability; a closely integrated but well-differentiated provider arm, with strong primary care; a degree of inter-sectoral co-ordination that serves as a model of good practice; detailed and effective dialogue between users and health service managers; and, innovation around professional roles and the use of ICT that other health systems could learn from. All this leads to health outcomes on a par with several OECD economies. Nevertheless, serious strains are evident: spending is rising steeply, fuelled by salaries, fees and facility payments based on last year's outlay. These spending increases are not always associated with improvement in services: waiting lists are excessively long and growing. The system is perhaps too stable: institutional rigidity and vested interests have stalled vital reforms, meaning that Costa Rica still lacks systematic application of DRGs and health technology assessment, despite attempts to bring them in.

Abstract

This report discusses the need for an integrated and cyclical approach to managing health technology in order to mitigate clinical and financial risks, and ensure acceptable value for money. The analysis considers how health systems and policy makers should adapt in terms of development, assessment and uptake of health technologies. The first chapter provides an examination of adoption and impact of medical technology in the past and how health systems are preparing for continuation of such trends in the future. Subsequent chapters examine the need to balance innovation, value, and access for pharmaceuticals and medical devices, respectively, followed by a consideration of their combined promise in the area of precision medicine. The final chapter examines how health systems can make better use of health data and digital technologies. The report focuses on opportunities linked to new and emerging technologies as well as current challenges faced by policy makers, and suggests a new governance framework to address these challenges.

Abstract

The State of Health in the EU country profiles provide a concise and policy-relevant overview of health and health systems in all EU countries, Iceland and Norway, emphasising the particular characteristics and challenges in each country. They are designed to create a means of mutual learning and voluntary exchanges that support the efforts of countries in their evidence-based policy making. Each country profile provides a short synthesis of: the health status in the country; the determinants of health, focussing on behavioural risk factors; the organisation of the health system; and the effectiveness, accessibility and resilience of the health system. The profiles are the joint work of the OECD and the European Observatory on Health Systems and Policies, in co-operation with the European Commission.
